Edward Simmons Massage Therapists LLC is seeking to increase our network database of Massage Therapists and Nail Technicians Nationwide. We are looking for Therapists in small or medium size cities who are licensed and
insured.

What do you need to qualify?
· A current copy of your Massage License.
· A copy of your certificate of graduation from a licensed Massage School.
· A copy of your Liability Insurance (main page).
· If you have your own business, a copy of your business papers including the federal employment identification number, if you do not have a company business, a social security number is needed.
· 3 personal or business referrals.

- Massage Bench with necessary oils and linen.
- Massage Chair.
- Transportation.
- Cell phone.
The application below needs to be completed and ‘Submitted’ both for Massage Therapists and Nail Technicians.
